Polish the index conversion routine a bit, fixing bug 5119.

We had been assuming too much about the format. Reference.lyx violated it, maybe because it is so old. But anyway, this is a bit more reliable.

Fix crash with Menu->Edit->Paste recent
As an added bonus, the selection will be adapted to current Buffer (for quites, etc).

Converters::convert(): following r25960 don't use FileName::operator==() as this didn't work in this case. Don't ask me why.

Fix http://bugzilla.lyx.org/show_bug.cgi?id=5097 with a hammer. The correct fix would be to call setBuffer() inside pasteSelectionHelper() only for those inserted paragraphs.
